Muromonab (also known as OKT3) is a murine monoclonal antibody that targets the CD3 epsilon (ε) chain of the T‑cell receptor (TCR) complex. It was the first monoclonal antibody approved for therapeutic use in humans (1986). By binding to CD3 on the surface of T cells, muromonab blocks TCR‑mediated signaling and initially activates then depletes peripheral T cells, leading to profound immunosuppression. It was historically used as an induction therapy to prevent acute rejection in solid organ transplantation (e.g., kidney, heart, liver). Due to its murine origin and associated side effects (e.g., cytokine release syndrome, human anti‑mouse antibody responses), it has been largely replaced by newer humanized or human anti‑CD3 antibodies and other immunosuppressants.
